This is a form that is intended to be used by Marquette University students, faculty, and staff who have dietary restrictions and need a specific meal prepared. Please fill out this form completely so that we may be able to assist you in the best way possible.

Please look at the menu for the day you are requesting a meal modification to let us know what menu item(s) need to be modified to fit your dietary needs.

Please order 1 - 2 hours prior to pick up

Please note, that meals not picked up 30 minutes after the given date and time will be charged to the person's account.

We are only accepting form entries that are submitted from 6:00 a.m. - 9:00 p.m.

If you are requesting food for the next day, please make sure to order within the appropriate order time frame (6am-9pm) or your request will be denied
